How Public Health Insurance Works in Germany

Germany's GKV system is built on the principle of solidarity: everyone pays according to their income, and everyone receives the same high-quality care. With around 95 public health funds (Krankenkassen) to choose from, the system offers both security and flexibility.

Understanding GKV Premiums & Contributions

The GKV contribution is calculated as a percentage of your gross income up to the Beitragsbemessungsgrenze (contribution ceiling) of €62,100/year in 2026. Income above this ceiling is not subject to additional health insurance contributions.

Base rate: 14.6% of gross salary (7.3% employee + 7.3% employer). On top of this, each Krankenkasse sets its own Zusatzbeitrag (additional supplement), averaging 2.5% in 2026. Your employer pays half of this supplement as well.

Family insurance: If your spouse earns less than €505/month (or €538/month from a mini-job) and your children have no independent income, they are covered free of charge under your GKV plan. This Familienversicherung has no time limit and applies regardless of family size.

JAEG threshold: Employees earning above €77,400/year (2026) may opt out of GKV and switch to private health insurance. Those below this threshold must remain in GKV but can freely choose between any of the approximately 95 available providers.

All employees earning below the Jahresarbeitsentgeltgrenze (JAEG) threshold of €77,400/year (2026) are required to join a public health insurance provider. Students, trainees, and recipients of certain social benefits are also mandatorily covered under GKV. Self-employed individuals and employees earning above the threshold can choose between GKV and private health insurance (PKV).

How much does GKV cost in Germany?

The standard GKV contribution rate is 14.6% of your gross salary, split equally between you and your employer (7.3% each). On top of this, each health fund charges an additional supplement (Zusatzbeitrag) averaging 2.5% in 2026. Your employer also pays half of this supplement.
